Treatment of abdominal trauma with serious perineal open wound

Objective To investigate the treatments of abdominal trauma with serious perineal open wound. Methods Ten cases of abdominal trauma with serious perineal open wound in our emergency traumatic surgery department of East Hospital from Jan. 2010 to May 2011 were collected, including 9 cases of pelvic fracture. The average age of the patients Was 37 years old（17 to 56）. Treatments included：①Anti-shock during the treatment of abdominal trauma; ②Appropriate and timely debridement;③Control of the damage and application of surgical treatment;④Anti-inflammatory therapy to prevent infection and interrupt the occurrence of MODS. Results Three patients died from hemorrhagic shock, the spread of infection and MODS. Seven cases healed satisfactorily with good functional recovery and quality of life. Conclusion The keys to treatments of abdominal trauma with serious perineal open wound are definite diagnosis and condition valuation, early anti-shock, essential emergency surgery with damage control principles and avoidance of the second attack. Early and thorough debridement, timely control of SIRS and blocking the occurrence of MODS are also important.
