血清CA125、HE4在卵巢癌中的诊断及鉴别诊断

目的探讨血清糖链抗原125(CA125)和人附睾蛋白4(HE4)水平对卵巢良恶性肿瘤的诊断意义。方法收集呼和浩特市第一医院妇产科2010年12月至2013年3月收治疗的卵巢癌患者90例(卵巢癌组)、卵巢良性肿瘤患者40例(卵巢良性组),采用化学发光法检测血清CA125,采用酶联免疫吸附法(ELISE)检测血清HE4,分析比较各组CA125及HE4血清水平。结果卵巢癌组CA125和HE4的血清水平显著高于卵巢良性组(P<0.05)。联合检测CA125和HE4与单独检测CA125的灵敏度、特异度、阳性预测值和阴性预测值比较差异均有统计学意义(P<0.05),说明联合检测优于常规单独应用CA125检测。结论血清CA125联合HE4检测卵巢癌的灵敏度均高于两者单独检测,对卵巢良恶性肿瘤的诊断与鉴别诊断有重要意义。

Diagnosis Values of CA125 and HE4 Assay in Serum of Patients with Ovarian Cancer

Objective To explore the diagnosis values of detecting the concentrations of carbohydrate antigen 125 （CA125） and human epididymal protein4（ HE4 ） protein in serum for patients with benign and malign ovarian tumor. Methods A total of 90 eases of patients with ovarian cancer（ ovarian cancer group）, as well as 40 cases of patients with benign ovarian tumor（ benign ovarian tumor group） and 60 cases healthy women（control group） admitted in Huhhot First Hospital from Dec. 2010 to Mar. 2013 were collected. The serum CA125 concentrations of the three groups were detected by chemiluminscance method. The serum HFA protein concentration of the three groups were detected by enzyme-linkde Immunosorbent Assay. The levels of CA125 and HEd- in each group were compared respectively. Results The levels of CA125 and HFA in serum of the ovarian cancer group were significantly higher than those of the benign ovarian tumorgroup and the control group ,the differences were statistically significant（ P 〈0.05 ）. The differences of detection sensi- tivity, specificity, positive predictive values and negative predictive values between the combined detection of CA125 and HE4 and single detection of CA125 were statistically significant（ P 〈 0.05 ）, suggesting that the combined detection of CA125 and HE4 was superior to the conventional single detection of CA125. Conclu- sion The sensitivity of the combined detection of serum levels of CA125 and HE4 is higher than the single detection of either of the above two indexes, therefore, the combined detection has great importance in the diagnosis and differential diagnosis of benign and malign ovarian tumors.
