2007～2012年阳城县农村地区癫痫防治管理项目效果评价

[目的]了解农村癫痫防治管理项目实施的效果。[方法]2012年5月,阳城县农村癫痫防治管理项目实施结束前1个月,对在访癫痫患者和家属进行调查,并与项目实施前(2007年7月)对同一人群同样内容的调查结果比较。[结果]调查苯巴比妥药物规范治疗的300例癫痫患者,年人均医疗费支出<600元者所占比例,管理期间为97.00%,管理前为25.67%(P<0.01);管理后各项癫痫认知和态度问题的正确率均较管理前明显提高(P<0.01)。癫痫发作时全身性抽搐、意识不清者所占比例和发作持续时间长者所占比例,管理前分别为94.33%、87.00%,管理治疗后分别为33.00%、11.33%(P<0.01);出现局部症状或先兆后意识丧失或全身抽搐者所占比例,日常工作正常者所占比例,生活满意者所占比例,管理前分别为41.33%、24.33%、33.00%,管理治疗后分别为63.00%、73.00%、93.00%(P<0.01)。管理期间治疗5、4、3年者的显效率分别为82.33%、73.68%、64.71%(P>0.05)。[结论]实施农村癫痫防治管理项目可提高癫痫患者对癫痫的认知水平,减轻医疗负担,癫痫发作症状明显好转。

Evaluation on Effect of the Rural Epilepsy Control Management Project,Yangcheng County,2007-2012

[Objective]To understand the effect of implementing the rural epilepsy control management project. [Methods]In May, 2012, the last month of before the completion of the rural epilepsy control management project implemented, a survey was conducted among the visited epileptic patients and their families,and compared with the same population before the project （July 2007） on the same content. [Results]A total of 300 cases of epileptic patients with phenobarbital drug standard treatment were surveyed,the proportion of annual per capita medical treatment expenditure less than 600 yuan was 97.00 %, but before the management the proportion was 25.67 % （ P 〉0.01）. The correct rate of epileptic cognition and attitude question significantly increased than before the management （ P 〉0.01）. The proportions of the seizure with systemic convulsion,unconsciousness and epilepsy prolonged attack were 94.33 % and 87.00 %, however before the implementation of the project the proportions were 33.00 % and 11.33 % （ P 〈0.01）. Before the managemet, the propor- tions of appearing local symptoms or aura symptoms then loss of consciousness or convulsion,normal daily work and life satisfaction respectively were 41.33%, 24. 33%, and 33.00%, after management the proportions were 63.00%, 73.00% and 93.00%（ P 〈0.01）. The significant efficiency for patients treated for 5 years, 4 years, 3 years was respectively 82.33 %, 73.680% and 64.71 % （ P 〈0.01）, [Conclusion]Implementation of the rural epilepsy control management project can improve the cognitive level of epileptic patients with epilepsy, reduce the medical burden, and markedly improve epilep tic seizure symptom.
