浙江衢州地区血流感染病原菌中真菌的分布和耐药性监测

摘    要:
目的 监测近 6 年来浙江衢州地区血流感染病原菌中真菌的分布和耐药性,为临床诊断和治疗提供依据。 方法 回顾性分析 2009 至 2014 年浙江衢州地区两家医院血培养标本中真菌分离率、菌种分布特点、科室分布及对抗真菌药物的耐药情 况。 结果 在统计年限内,两家医院血培养标本中共分离真菌 126 株,占所有血培养检出菌的 5.8%。其中白色假丝酵母菌 48 株 (38.1%),热带假丝酵母菌 29 株(23.0%),近平滑假丝酵母菌 26 株(20.6%);科室以重症监护中心最多;分离的真菌对 5 种抗真菌药 物的敏感率分别为:5- 氟胞嘧啶(100.0%)、两性霉素 B(100.0%)、氟康唑(97.6%)、伏立康唑(98.4%)、伊曲康唑(87.3%)。 结论 血流感染真菌种类复杂,以假丝酵母菌为主,对常用抗真菌药物具有较高的敏感性,在血培养检出菌中占有一定的比例,临床应加强对 血培养真菌的检测。

Abstract:
Objective To investigate the distribution and drug resistance of fungal isolated from patients with bloodstream infection in Quzhou city of Zhejiang province. Methods The distribution and drug resistance of fungal strains isolated from blood culture specimens from 2009 to 2014 in Quzhou city were retrospectively analyzed. Results Total 126 fungal strains(5.8%) were isolated, including 48 strains of Candida albicans (38.1%), 29 strains of Candida tropicalis (23.0%) and 26 strains of Candida parapsilosis (20.6%). Most of the isolates were isolated from blood samples collected in ICU. The susceptibilities of isolated strains to 5 kinds of antifungal drugs were 5-fluorocytosine (100.0% ), amphotericin B (100.0% ), fluconazole (97.6% ), voriconazole (98.4%) and itraconazole (87.3%), respectively. Conclusion Candida were most common fungal species isolated from bloodstream infection in Quzhou region, and most of the isolates are highly sensitive to commonly used antifungal agents.
