鼻内窥镜下翼管神经切断术治疗过敏性鼻炎

目的：研究鼻内窥镜下切断翼管神经的可行性。方法：应用统计学方法比较三种切断翼管神经方法治疗过敏性鼻炎的疗效、副作用和可操作性。（1）经上颌窦途径切断翼管神经；（2）鼻内窥镜下切断翼管神经；（3）鼻内窥镜下切断翼管神经配合筛前神经电灼或下鼻甲或鼻中隔矫正术。结果：鼻内窥镜下切断翼管神经在术后反应、手术可操作性方面明显优于经上颌窦法，但疗效无明显差异。若配合筛前神经电灼并下鼻甲部分切除或鼻中隔矫正术，则手术效果有所提高。结论：鼻内窥镜下切断翼管神经较经上颌窦途径更简捷、准确，可同时配合下鼻甲、鼻中隔手术或筛前神经电灼术以提高疗效。

Treatment of Allergic Rhinitis by Cutting the Vidian Nerve under the Nasal Endoscope

Objective: To study the possibility cutting the vidian nerve under the nasal endoscope.Methods:The effects, complications and practicability of three cutting methods were compared statistically. The first is trans maxillary sinus cutting; the second is trans nasal endoscope cutting; the third is trans nasal endoscope cutting combined with ethmoid nerve cauterization and nasal septum correction or the partial cut of the infraturbinal.Results:The second method is superior to the first one in terms of postoperative reaction and practicability, but there is no significant difference in cure rate between them. The third method is the best one.Conclusion:The method of cutting the vidian nerve under the nasal endoscope is simpler and more accurate than the trans maxillary sinus cutting. For improving the curative effect, ethmoid nerve cauterization and nasal septum correction or the partial cut of the infraturbinal can be performed during the endoscopic operation.
