| 摘 要： | 目的 探讨BiPAP模式在老年重症全麻术后患者机械通气中的应用价值.方法 将老年重症全麻术后患者随机分为BiPAP组和VC-SIMV组,同步动态观察两组的临床疗效、呼吸力学、血流动力学、动脉血气、机械通气时间、镇静剂用量等指标,比较BiPAP模式和VC-SIMV模式对上述指标的影响.结果 BiPAP组ICU病死率(26.09%)低于VC-SIMV组(50.00%,P＜0.05);两种模式在改善动脉血气指标方面均有明显效果(P＞0.05);BiPAP模式在改善呼吸力学指标方面优于VC-SIMV模式(P＜0.05);对血流动力学的影响,BiPAP模式优于VC-SIMV模式(P＜0.05);BiPAP组和VC-SIMV组机械通气时间为(163.79±22.85)h和(356.23±32.65)h,两组比较差异有统计学意义(P＜0.05);围机械通气期BiPAP组和VC-SIMV组的镇静剂用量分别为(95.26±17.28)mL 和(306.13±61.75)mL,两组比较差异亦有统计学意义(P＜0.05).结论 对老年重症全麻术后患者进行机械通气时,BiPAP模式优于VC-SIMV模式,BiPAP模式可作为老年重症全麻术后患者首选的机械通气方法 . |
