摘    要:目的 探讨急性脑实质内出血的磁共振扩散加权成像(DWI)表现及其发生机制。方法 对临床症状出现后12h内行DWI检查的17例急性脑实质内出血病人的DWI及ADC图进行观察分析。结果 17例急性脑出血中,在DWI上9例表现为中央低信号,周边不规则高信号;6例表现为中央不均质高信号,周边见低信号;2例表现为高、低混杂信号。在b=0时的基础采集像上11例表现为明显的低信号,其中6例于低信号区内可见间杂的等、高信号;6例表现为等信号,周围见不规则的低信号环。在ADC图上15例表现为不均质的低信号。结论 结合ADC图及b=0的基础采集像,DWI能够对急性脑实质内出血作出准确诊断。

Abstract:Objective To investigate the manifestations and mechanism of acute intracerebral hemorrhage on diffusion-weighted MR imaging(DWI). Methods DWI was performed for patients with acute intracerebral hemorrhage(n=17) within 12 hours after the onset of symptoms.Both the DWI and ADC map were analysed to investigate the manifestations and the mechanism of the disease. Results On DWI,among the 17 patients,nine were found with low signal in the centre and irregular outer high signal,six with middle inhomogeneous high signal and surrounding low signal,and two with mixed signal.On the DWI(b=0),obvious low signals were found in 11 patients of whom iso-or high signals were also found in the low signal area in six patients,in isointensity with surrounding irregular low signal.Fifteen patients with inhomogeneous low signal were found on ADC map. Conclusion Combined with ADC map and the DWI(b=0),accurate diagnosis of acute intracerebral hemorrhage could be made by diffusion-weighted MR imaging.
