临床药师在肿瘤内科开展药学查房的工作模式探讨

目的:探索临床药师在肿瘤内科开展药学查房的工作模式。方法:介绍临床药师参与查房,了解化疗患者、发生不良反应患者、合并基础疾病患者的基本情况,总结与完善肿瘤内科药学查房的内容和方法。结果:临床药师通过参与药学查房进行化疗反应调查和对患者进行药学咨询与教育,可及时发现问题,优化化疗药及辅助药的使用。结论:在肿瘤内科进行药学查房,可减少肿瘤患者不良反应的发生,有助于提高患者用药的安全性、有效性及依从性。

Exploration of Working Pattern of Clinical Pharmacists Developing Pharmaceutical Ward Round in Oncology Department

OBJECTIVE： To explore the working pattern of clinical pharmacists developing pharmaceutical ward round in oncology department. METHODS： Clinical pharmacists participated in ward round and investigated the physical condition of patients who received chemotherapy, had adverse drug reaction and accompanied with other diseases. The contents and methods of pharma- ceutical ward round in oncology department were summarized and improved. RESULTS： Clinical pharmacists carried out chemotherapy reaction survey and pharmaceutical consultation and education for patients by participating in pharmaceutical ward round, which found out problems timely and optimized the use of chemotherapeutic drugs and adjuvant drugs. CONCLUSIONS： Standard pharmaceutical ward round in oncology department can reduce the adverse drug reaction in tumor patients and improve the safety, effectiveness and compliance of drug use.
