血清ANCA检测在原发性小血管炎中的临床意义

目的 检测原发性小血管炎患者血清抗中性粒细胞胞浆抗体(ANCA)并探讨其临床意义.方法 应用间接免疫荧光(IIF)法检测50例原发性小血管炎疾病患者血清中ANCA的检出率.结果 50例原发性小血管炎血清中,韦格纳肉芽肿(WG)阳性率95%,为C-ANCA阳性;显微镜下多血管炎(MPA)阳性率50%,为P-ANCA阳性;过敏性紫癜(AP)阳性率37.5%,为P-ANCA阳性;白塞病(BD)3例,ANCA均为阴性;未分型原发性小血管炎(UCPSV)阳性率66.67%,其中C-ANCA 4例,P-ANCA 6例.结论 抗中性粒细胞胞浆抗体在原发性小血管炎中,部分疾病与抗中性粒细胞胞浆抗体密切相关,尤其是韦格纳肉芽肿阳性率95%,提示C-ANCA可作为韦格纳肉芽肿血清学标志性抗体,ANCA检测对血管炎的发病及诊断具有一定的作用.

Clinical significance of detecting serum ANCA in Primary small vasculitis

Objective To disscuss the clinical significance of antineutrophil cytoplasmic antibody(ANCA) in primary small vasculitis.Methods The serum ANCA was tested by indirect immuno-fluorescence(IIF) in 50 patients.Results The positive rate of serum ANCA in wegner's granulomatosis(WG) was 95%,it was C-ANCA positive;50% in microscopicpoly angiitis(MPA),which was P-ANCA positive;37.5% in anaphylactoid purpura(AP),which was P-ANCA positive;66.7% in un-classed primary systemic vasculitis(UCPSV),of which 4 was C-ANCA positive,6 was P-ANCA positive;the ANCA in 3 behcet's disease(BD) were negative.Conclusion The detection of serum ANCA showed the important diagnostic significance in ANCA associated small vasculitis,especially in WG,the C-ANCA can be regarded as a serum marker of diagnose it.
