校正改良早期预警评分对重症急性胰腺炎患者病情评估的价值

目的探讨校正改良早期预警评分(MEWS)对重症急性胰腺炎(SAP)患者病情评估的应用价值。方法以患者接诊后30d的结局作为预后结果,对42例SAP患者同时进行校正MEWS评分和MEWS评分。结果存活(30例)和死亡(12例)SAP患者的校正MEWS评分均明显高于MEWS评分[(4.7±1.6)分和(12.9±2.3)分vs.(2.9±1.2)分和(8.2±1.8)分](P<0.05)。以患者死亡作为预测指标,校正MEWS评分的受试者工作特征(ROC)曲线下面积明显大于MEWS评分(P<0.05)。校正MEWS评分的敏感度为100.0%,特异度为4.7%;MEWS评分的敏感度为87.2%,特异度为11.3%。结论与MEWS评分比较,校正MEWS能更准确反映SAP患者病情危重程度和预测SAP患者的病死率。

Role of corrected modified early warning score in evaluating the severity of patients with severe acute pancreatitis

Objective To investigate the application value of corrected modified early warning score(MEWS) in evaluating the severity of patients with severe acute pancreatitis(SAP).MethodsTaking the endings on 30 days after admission as the prognostic results,forty-two SAP patients were evaluated by corrected MEWS and MEWS at the same time.Results The scores given by corrected MEWS were higher than those by MEWS in SAP patients who were survival(30 cases) and died(12 cases)(4.7±1.6) points and(12.9±2.3) points vs.(2.9±1.2) points and(8.2±1.8) points(P<0.05).Taking death as a predictor,the area under ROC curve was greater calculated by corrected MEWS than that calculated by MEWS(P<0.05).The sensitivity and specificity of corrected MEWS were 100.0%,and 4.7%,respectively,which were significantly different from 87.2% and 11.3% of MEWS(P<0.01).Conclusion Compared with MEWS,the corrected MEWS is more accurate in reflecting the severity and predicting the mortality for the patients with SAP.
