外伤性脑疝术后脑梗死早期CT表现及CT诊断价值

目的探讨分析螺旋CT在外伤性脑疝术后脑梗死诊断中的应用价值。方法择取该院收治的37例外伤性脑疝术后并发脑梗死患者的临床诊断资料进行回顾性分析,探讨术后CT检查在诊断脑梗死中的临床应用价值。结果术后24 h复查检出脑梗死6例,24-48 h检出13例,3-6 d检出18例,大面积梗死21例,小灶性梗死16例;5例患者强化CT结果显示为大脑动脉或分支动脉狭窄、闭塞,后期复查出现梗死灶;预后良好率62.16%。结论豆状核轮廓模糊、脑动脉致密血管征、岛带及基底核消失征等CT征象是早期脑梗死的重要指标,临床重视CT影像早期改变,提高早期诊断率。

CT Findings and CT Diagnostic Value of Early Cerebral Infarction after Traumatic Cerebral Hernia Surgery

Objective To investigate the application value of spiral CT in the diagnosis of cerebral infarction after traumatic cerebral hernia surgery. Methods The clinical diagnostic data of 37 cases of traumatic hernia surgery with cerebral infarction admitted in our hospital from January, 2008 to June, 2013 were analyzed retrospectively. The clinical application value of postoperative CT detection in the diagnosis of cerebral infarction was investigated. Results 6 cases of cerebral infarction were detected in the reviewing 24 h after the operation, 13 cases were detected in the reviewing 24-48 h after the operation, in the reviewing 3 to 6 days after the operation, 18 cases were detected, 21 cases of large area of infarction, 16 cases of small focal infarction; enhanced CT of5 cases showed cerebral artery or branch artery stenosis, occlusion, infarct appeared in post-review; the good prognosis rate was62.16%. Conclusion CT signs of lenticular nucleus vague outline, dense vascular symptoms of cerebral arteries, insular ribbon and basal ganglia disappearance are important indicators of early cerebral infarction. Emphasis on early changes in CT images can improve the rate of early diagnosis.
