摘    要:
目的 探讨不同类型胃息肉与幽门螺杆菌感染的关系。方法 经胃镜活检及病理诊断胃息肉106例,内镜下摘除的胃息肉作HE染色,判定胃息肉组织学类型及炎症反应程度。同时在胃窦部取材2~3块,分别作快速尿毒酶试验及美蓝染色,以检测Hp感染率,两者均阳性判为Hp感染。结果 106例胃息肉Hp总检出率为65.1%。胃息肉以增生息肉多见,共76例,占71.7%,腺癌性息肉30例,占28.3%。Hp感染率在增生性息肉中为73.7%(56/76),显著高于腺癌性息肉43.1%(13/30)(P<0.01)。活动性炎症在增生性息肉中为55.3%(42/76),显著高于腺瘤性息肉 0%(9/30)(P>0.05)。结论 增生性息肉的发生可能与Hp感染有关,这可能与Hp引起胃粘膜活动性炎症反应有关。

Abstract:
Objective To investigate the relationship between Helicobacter pylori(H. pylori) infection and gastric polyps in dif-ferent types. Methods 106 cases of gastric polyps were diagnosed by gastroscope and histopathothologic examination, the polypsobtained by gastroscope were HE stained to determine the histological types and the level of inflammatory response. At the same, 2or 3 gastric mucosa biopsy specimen were taken from gastric antrum and rapid urea enzyme test and methylene blue staining wasmade to detect H. pylori infection: if both tests showed positive, the infection would be conformed by H. pylori. Results Out of the106 cases with gastric polyps, HP infection took up 65.1% 76 cases(71.7%) were hyperplastic polyps, 30 cases(28.3%) wereadenomaous polyps. THe H. pylori infection rate in hyperplastic polyps (56/76; 73.7%) was significantly higher than adenomaouspolyps(13/30; 43.1%) (P<0.01). Active inflammatory response(42/76; 55.3%) was significantly ghigher in hyperplastic polypthan in adenomaous polyp(9/30; 30%) (P>0.05). Conclusion The development of hyperplastic polyps is associated with H.pylori infection, which may be related to the active inflammatory response caused by H. pylori.
