新辅助化疗对进展期胃癌手术并发症的影响

目的 探讨新辅助化疗对进展期胃癌手术并发症的影响。方法 随机数字表法将2009年9月—2013年7月在福建医科大学附属第一医院胃肠外科接受治疗的145例进展期胃癌患者，分为新辅助化疗组（62例）和直接手术组（83例）。比较两组病例术后并发症发生情况。结果 两组病例术前一般情况比较差异无统计学意义（P＞0.05）。两组术后主要并发症发生率的差异无统计学意义(P＞0.05)。化疗组术后第7天白细胞水平显著高于术前及同期常规手术组，两组比较差异有统计学意义（P＜0.05）。新辅助化疗组的手术根治率高于常规手术组，两组比较差异有统计学意义（P＜0.05）。结论 新辅助化疗不会增加手术并发症的发生率，并可提高手术根治率。

Influence of Neoadjuvant Chemotherapy on Operative Complications of Advanced Gastric Carcinoma

Objective To explore the effects of neoadjuvant chemotherapy on the operative complicationsof advanced gastric carcinoma. Methods According to random number table method, 145 patients withadvanced gastric carcinoma hospitalized in the Gastrointestinal Surgery Department of The First AffiliatedHospital of Fujian Medical University from Sep 2009 to Jul 2013 were randomly divided into two groups,one group (62 cases) received neoadjuvant chemotherapy and surgery, and the other group (83 cases)received conventional surgery. Operative complications in two groups were compared. Results Betweenthe two groups, the differences of general conditions before operations and incidence rate of main operativecomplications had no statistical significance(P>0.05). Postoperative white blood cells of neoadjuvantchemotherapy group after 7 days were significantly higher than those of preoperative and correspondingperiod of conventional surgery group, with statistically significant difference(P＜0.05). Operation cure rateof neoadjuvant chemotherapy group was higher than that of conventional surgery group, with statisticalsignificance(P＜0.05). Conclusion Neoadjuvant chemotherapy is helpful to improve operation cure ratewithout increasing the risk of postoperative complications.
