摘    要:目的探讨D-二聚体在诊断下肢深静脉血栓(DVT)形成中的临床意义。方法采用免疫比浊法测定110例DVT可疑患者及50例健康体检者血浆中D-二聚体含量。并对其作为诊断性实验指标的临床价值进行分析评价。结果DVT可疑组10例及对照组50例患者的血浆D-二聚体低于临界值水平,上述60例经下肢深静脉造影检查排除DVT;DVT可疑组剩余100例患者D-二聚体高于临界值,经下肢深静脉造影检查确诊DVT患者84例;血浆D-二聚体检测对于DVT诊断的敏感性、特异性、阳性预测值及阴性预测值分别为100%、38.5%、84.0%和100%。结论血浆D-二聚体检测可作为排除DVT的首选筛查试验,值得在临床诊断中推广应用。

Abstract:Objective To evaluate the value of plasma D-dimer in diagnosing deep venous thrombosis (DVT). Methods Immune turbidimetry was used to detect the D - dimer levels of 110 cases of patients who suspected with lower ex-tremity deep venous thrombosis and 50 healthy subjects. Compared and analyzed the D - dimer levels as a critical diagnostic test evaluation standards. Results The plasma D - dimer levels in 50 cases of healthy persons and 10 cases among the 110 ca-ses of suspected DVT were lower than the critical value, which were excepted from DVT by deep vein angiography of Lower limbs ; Among the other 100 cases whose plasma D - dimer level were higher than the critical value ,84 cases were confirmed hav-ing DVT by deep vein angiography examination;plasma D - dimer as DVT determining critical standards, the sensitivity, specific-ity,positive predictive value,negative predictive value were 100%, 38. 5%, 84. 0% and 100% respectively. Conclusion De-termination of D - dimer could be applied as exclusion test in diagnosis of DVT and is worthy of oooularization in clinic.
