合并不同高密度脂蛋白胆固醇水平早发冠心病患者临床类型及冠状动脉病变关系分析

目的探讨合并不同血清高密度脂蛋白胆固醇（HDL-C）水平早发冠心病患者临床类型及冠状动脉病变的特点。方法入选100例因胸痛或胸闷不适行冠状动脉造影并确诊为早发冠心病的患者。以HDL-C水平分为两个亚组，并对冠脉病变程度进行分型，与100例非冠心病对照组进行比较分析。结果早发冠心病组血清HDL-C（1．07±0．27mmol／L）明显低于对照组（1．16±0．26mmol／L，P〈0．05）；早发冠心病患者中HDL-C低于0．9mmol／L亚组中，临床表现为急性冠脉综合征的比例高（69．2％），且明显高于稳定型心绞痛组（23．9％，P〈0．05）。冠状动脉为C型病变组HDL-C明显低于A、B型病变组（P〈0．05）。结论早发冠心病患者血清HDL-C水平与临床类型及冠状动脉病变类型密切相关。

Relationship Between Serum HDL-C Levels and Clinical Types and Coronary Lesions in Patients with Premature Coronary Heart Disease

Objective To explore the relationship between serum HDL-C levels and clinical types and coronary lesions in patients with premature coronary heart disease. Methods 100 cases with chest pain or chest discomfort diagnosed as premature coronary heart disease undergoing coronary angiography were divided into two subgroups according to HDL-C levels. The clinical types and severity of coronary lesions were analyzed. Results Serum HDL-C levels in premature coronary heart disease group were significantly lower than that in control group [ （ 1.07±0.27 ） mmol/L vs （ 1.16±0.26 ） mmol/L, P 〈0. 05]. Patients with lower HDL-C（ 〈0. 9 mmol/L） suffered more with acute coronary syndrome. Patients with C type of coronary lesion had lower HDL-C levels than those with A and B types of coronary lesions. Conclusion Patients with premature coronary heart disease had lower HDL-C levels and HDL-C levels are closely related with its clinical types and types of coronary lesions.
