坐位颅脑手术的麻醉处理

摘    要:目的 研究如何减轻坐位颅脑手术病人的生理变化。方法  2 0例病人均采用体位改变前扩容、半清醒插管、体位改变后进行控制呼吸等方法。监测心率、血压、血氧饱和度及尿量的变化。结果 患者在体位改变及麻醉前后心率、血压、血氧饱和度等生理指标改变不明显。结论 采用半清醒插管、体位改变前扩容等措施可减少体位改变对机体的不良影响。

Abstract:Objective To Study how to reduce the physiologic change during posterior fossa operation in sitting position. Methods Twenty patients were administered balanced salt solution 20mg/(kg·h) and gelofusine 20mg/(kg·h) before posture change, the laryngeal pharynx and trachea were given superficial anesthesia; Diazepam 0.2mg/kg was infused intravenously , after posture change the patients were given controlled respiration. The heart rate, blood pressure, SPO-2 and urine volume were monitored intraoperatively. Results No significant changes were found in HR, MAP, SPO-2 and other physiological indices either between the posture shifts or before and after the administered anesthesia. Conclusion The combination of increasing volume before posture change , intubating in semiconsciousness and control respiration after posture change may reduce the physiological effect and improve the safety of anesthesia during posterior fossa operation in sitting position.
