外侧切口保留关节囊的老年人工股骨头置换

目的介绍一种人工股骨头置换的改进方法。方法2003年3月～2006年3月，采用外侧切口，保留髋关节囊，对33例老年股骨颈骨折患者行人工股骨头置换术。结果33例患者手术时间为30～70min，平均45min。术中最多出血量为250ml，平均出血量为100ml。术后无切口感染及髋关节脱位发生，所有患者经8个月至3年随访，平均1年10个月.均无髋关节脱位、松动发生。术后3、6个月随访时采用Harris髋关节功能评分，分别为90．7分和92．2分。随访8个月至3年，测量双下肢长度患肢比健肢平均长0．2em（范围0～0．7cm）。结论外侧切口保留关节囊的老年人工股骨头置换术是一种安全、有效的髋关节功能重建的治疗方法，值得提倡。

Femoral head arthroplasty reserving articular capsule by lateral incision to treat femoral neck fracture of elderly patients

Objective To introduce a method of femoral head arthroplasty reserving articular capsule to treat elderly patients with acute femoral neck fracture.Methods From March 2003 to March 2006,33 elderly patients with femoral neck fracture were treated with femoral head arthroplasty reserving articular capsule by lateral incision.Results The operation time was from 30 to 70 minutes(in average of 45 minutes).The most blood lose was 250 ml(in average of 100 ml).There were no complications of infection and dislocation after operation.All the cases had been followed up from 8 months to 3 years(in average of 1 year and 10 months),no dislocation and prosthetic loose happened.The scores was 90.7 and 92.2 respectively after 3 and 6 months follow up according to Harris hip joint score.The average length of the affected limb is 0.2 cm(range 0-0.7 cm) longer than that of the healthy limb.Conclusion Femoral head arthroplasty reserving articular capsule by lateral incision to treat femoral neck fracture of elderly patients is a safety,convenient and good method.
