136例青年胃癌患者预后影响因素临床统计分析

目的：分析青年人胃癌的临床病理特点与预后之间的关系。方法：对解放军总医院1998年至2008年间136例40岁以下（含40岁）青年胃癌患者（其中男性90例,女性46例）,按肿瘤部位、大体类型、组织学类型、浸润深度、TNM分期、生长方式及转移情况进行比较分析,并对所有病例进行随访。结果：121例获得随访结果,结果显示影响青年人胃癌生存率的因素：年龄（RR=0.965）、性别（RR=0.904）、脉管癌栓（RR=0.978）、嗜酸性粒细胞（RR=0.952）、肿瘤浸润深度（RR=0.967）、肿瘤最大径（RR=0.979）、淋巴结转移（RR=0.982）、远处转移（RR=0.974）、临床分期（RR=0.911）等均P〈0.05,其余因素未有显著的统计学差异。结论：影响青年胃癌人胃癌生存率的主要因素有年龄、性别、脉管癌栓、嗜酸性粒细胞、肿瘤浸润深度、肿瘤最大径和淋巴结转移,具有统计学意义。

Clinical and Statistical Analysis of Prognosis Impacting Factors of 136 Youth Cases with Gastric Cancer

Objective：To investigate the relationships between the prognosis impacting factors and the clinical pathology characteristics of youth cases with gastric cancer.Method：One hundred and thirty six youth cases（age≤40,male=90,female=46） with gastric cancer in PLA General Hospital from 1998 to 2008 were observed with follow-up,comparison and analysis were made according to their tumor site,type on the whole,histology type,infiltration extent,TNM staging,growth pattern and metabasis level.Result：One hundred and twenty one cases could be followed up.The main survive rate impacting factors of youths with gastric cancer were age RR=0.965,gender RR=0.904,vessel tubular cancer clot RR=0.978,eosinophile granulocyte RR=0.952,tumor infiltration extent RR=0.967,tumor max diameter RR=0.979 and lymphaden metabasis RR=0.982,distant metabasis RR=0.974,clinical stage RR=0.911,all of above with P0.05,other factors posses no obvious statistical variance.Conclusion：The main survive rate impacting factors of youths with gastric cancer may include age,gender,vessel tubular cancer clot,eosinophile granulocyte,tumor infiltration extent,tumor max diameter and lymphaden metabasis,any of them posseses obvious statistical implication.
