新诊断2型糖尿病患者血浆脂肪甘油三酯脂酶与脂代谢、胰岛素抵抗的相关性研究

目的：探讨新诊断2型糖尿病（T2DM）患者血浆脂肪甘油三酯脂酶（ATGL）水平变化与脂代谢、胰岛素抵抗（IR）的关系。方法：选取新诊断T2DM患者60例,正常对照（NC）组30例。观察其血浆ATGL、TG、HDL等相关水平的变化,并计算其胰岛素抵抗指数（HOMA-IR）、胰岛素敏感性（ISI）等指标。结果：（1）血浆ATGL与高密度脂蛋白（HDL）、ISI呈正相关（P〈0.05）;与甘油三酯（TG）、空腹血糖（FPG）、Fins（空腹胰岛素）、体重指数（BMI）、胰岛素抵抗指数（HOMA-IR）、呈负相关（P〈0.01）。（2）多元逐步回归分析发现：HOMA-IR、ISI、HDL作为独立危险因素影响ATGL水平,HOMA-IR为影响ATGL最显著的因素。结论：血浆ATGL的水平表达减低,存在甘油三酯水解的异常,可能改善IR。

Newly diagnosed type 2 diabetic patients with adipose triglyceride lipase and lipid metabolism , insulin resistance research

Objective:To explore the newly diagnosed type 2 diabetes(T2DM)patients with adipose triglyceride lipase(of ATGL) levels and lipid metabolism,insulin resistance(IR) in the relationship.Methods:Select 60 cases of the newly diagnosed patients with T2DM,30 cases of normal control(NC) group.Observe the changes in the plasma of ATGL,TG,HDL level,and calculate insulin resistance index(HOMA-IR),insulin sensitivity index(ISI).Results:(1)The plasma ATGL and high-density lipoprotein(HDL),the ISI was positively correlated(P<0.05),and triglyceride(TG),fasting plasma glucose(FPG)of Fins(fasting insulin),body mass index(BMI),insulin resistance index(HOMA-IR) was negatively correlated(P<0.01).(2)Multiple stepwise regression analysis find that: HOMA-IR,ISI,HDL was independent risk factors.ATGL level.and HOMA-IR is the most significant factors to affect ATGL.Conclusion:The level of plasma ATGL expression reduced and the exception of triglyceride hydrolysis may improve IR.
