17例误诊为肺癌的肺曲霉菌病的临床分析

目的探讨肺曲霉菌病的临床表现和影像学特点。方法回顾性调查对我院自2000年1月-2007年7月确诊的肺曲霉菌病进行分析研究。结果肺曲霉菌病的症状主要表现为咳嗽16例（94．17％）；咯白粘痰12例（70．58％）；发热12例（70．58％）；胸闷气短6例（35．29％）；咯血痰5例（29．41％），干咳2例（11．76％）。影像学改变主要表现为结节样病灶（6例，占35．29％），空洞样病灶（3例，占17．65％），块样球型病灶（2例，占11．76％）；CT增强扫描明显强化者6例，占35．29％。确诊方法：外科手术后病理检查16例，纤维支气管镜下活检1例。结论肺曲霉菌病的症状不典型，影像学改变不典型。病理组织活检具有重要的诊断价值。

Clinical analysis of 17 pulmonary asperqillosis cases misdiagosed as pulmonary carcinoma

Objective To analyze the clinical manifestations and imaging features of pulmonary aspergrillosis. Methods The 17 pulmonary aspergrillosis cases misdiagnosied as lung cancer during 2000 and 2007 were analyzed. Results All the clinical manifestations of lung aspergrillosis were non - specific, and the main manifestations were cough（ 94.17% ）, white phlegm （70.58%）, fever （70.58%）, dyspnea （35.29%）, hemoptysis （29.41%）, hacking cough（ 11.76% ）. The fea-tures of chest x - ray were also non - specific including nodus focus （35.29%）, cavity focus（ 17.65% ）, and cystlike mass （ 11.76% ）. Conclusion The clinical manifestations and the features of chest x-ray of pulmonary aspergrillosis are non-specific and pathological biopsy aids in daignosis.
