目的比较牙冠延长术与牙龈切除术用于残根残冠桩冠修复中的疗效。方法选择2006—2008年大连大学附属中山医院口腔科收治的因外伤或龋坏致患牙断面达龈下的患者50例(58颗患牙),按照入院先后顺序随机分为两组,分别采用牙龈切除术(30颗牙)与牙冠延长术(28颗牙)完全暴露断面,6周后行常规桩冠修复,评价两组在术后2周的疗效及桩冠修复后6个月的牙周健康状况。结果术后2周,牙冠延长术组总有效率为96.67%(29/30),牙龈切除术组总有效率为71.43%(20/28),两组比较差异有统计学意义(P<0.05);桩冠修复后6个月,牙冠延长术组总有效率为93.33%,牙龈切除术组总有效率为64.29%,两组比较,差异亦有统计学意义(P<0.05)。结论牙冠延长术能有效恢复患牙生物学宽度,有利于修复后牙周健康的维持。

Comparison of the restorative effects of crown lengthening surgery and gingivectomy on dowel crown of deep-seated subgingival teeth fracture

Abstract：Objective To compare the restorative effects of crown lengthening surgery and gingivectomy on dowel crown of deep-seated subgingival teeth fracture. Methods Fifty-eight teeth were selected and divided into the crown lengthening surgery group and gingivectomy group .We performed regular restoration of post crown after the teeth cross-section was completely exposed. Then， the clinical effects were compared by measuring the depths of gingival pocket at the time of two weeks and six months after operation， and the results were evaluated by χ2 test. Results Two weeks after operation， the effective rate in the gingivectomy group was much lower than the rate of the crown lengthening surgery group （P ＜0.05） . Six months after operation，the occurrence rate of periodontitis in the gingivectomy group was significantly higher than that in the crown lengthening surgery group （ P ＜0.05）. Conclusion According to the biologic width principle， crown lengthening surgery can conserve these teeth with subgingivally involved defect， and has a good， long-time clinical effect.
