单开窗棘突切除术治疗中央型腰椎间盘突出症

摘    要:
目的探讨一种治疗中央型腰椎间盘突出症的手术方法和疗效。方法在一侧椎板开窗术的基础上,皎除相应棘突,保留棘上韧带,并将韧带牵向对侧,使椎管充分显露,摘除中央突出之椎间盘。结果单开窗棘突切除术治疗中央型腰椎间盘突出症32例,随访1—5年,优20例,良10例,可2例,优良率93、75%。结论单开窗棘突切除术操作方便,术中昆露清晰,既达到椎管减压、摘除突出之椎间盘的目的,又保留了对侧椎板、棘上韧带,维持了后柱稳定,是目前治疗巾央型腰椎间盘突出症理想的手术方式。

Abstract:
Objective To explore a operation approach and its curative effect to the central protrusion of lumbar intervertehral disc. Methods The corresponding spinous process was resected on the basis of fenestration to one side, but supraspinous ligament should be retained and retracted to the opposite side, and then diskeetomy had been done. Results 32 patients of central protrusion of lumbar intervertebral disc were treated with the is method and followed - up for 1 to 5 years. 20 patients were regarded as excellent and 10 good . The total percentage of excellent and good was 93.7%. Conclusion Fenestration with the spinous process resection was easy to operate, had a clear range . This method not only had advantage of decompression and diskectomy, but reserved opposite lamina,supraspinous ligament, and maintained the stability of posterior vertebral column. So the method should be a good approach to the central protrusion of lumbar intervertebral disc.
