经皮椎间盘摘除术与联合三氧注射治疗腰椎间盘突出症的疗效比较

目的 比较经皮椎间盘摘除术联合三氧注射与单纯经皮椎间盘摘除术和单纯三氧椎间盘注射术的治疗效果.方法 腰椎间盘突出症患者215例,分为经皮腰椎间盘摘除术联合三氧注射治疗组（A组）73例,单纯经皮椎间盘摘除术治疗组（B组）68例和单纯三氧椎间盘注射术治疗组（C组）74例.术后1、3、6个月时随访.结果 术后1个月时,A组显效50.7%（37/73例）,有效39.7%（29/73例）,无效9.6%（7/73例）,总有效率90.4%（66/73例）;B组显效33.8%（23/68例）,有效38.2%（26/68例）,无效27.9%（19/68例）,总有效率72.1%（49/68例）;C组显效27.0%（20/74例）,有效41.9%（31/74例）,无效31.1%（23/74例）,总有效率68.9%（51/74例）.三组比较差异有统计学意义.术后3个月和6个月A组和B组疗效比较差异无统计学意义,A组、B组与C组疗效比较差异有统计学意义.结论 经皮椎间盘摘除术联合三氧注射治疗腰椎间盘突出症是一种安全、有效的治疗方法,效果优于单纯经皮锥间盘摘除术与单纯三氧注射术.

Comparising the efficacy of percutaneous discectomy combined with or without ozone injection in the treatment of lumbar intervertebral disc herniation

Objective To compare the efficacy of percutaneous discectomy combined with ozone injection with only percutaneous discectomy or only ozone injection in the treatment of lumbar intervertebral disc herniation. Methods Two hundred and fifteen patients with lumbar intervertebral disc herniation were randomly divided into three groups. Group A （n=73） was treated by percutaneous lumbar discectomy combined with ozone injection; Group B （n =68） only by simple percutaneous disceetomy; Group C （n =74） only by ozone injection. All patients were fol lowed up at 1 month, 3 months and 6 months after the treatment. Results The excellent,effective and ineffective efficacy rates were 50.7%（37/73 cases）,39.7%（29/73 cases） and 9.6%（37/ 73 cases） respectively and the total efficacy rate was 90.4（66/73 cases） in group A 1 month after the treatment. The excellent, effective and ineffective efficacy rates were 33.8% （23/68 cases）, 38.2%（26/68 cases） and 27.9%（19/68 cases） respectively and the total efficacy rate was 72.1 （49/68 cases） in group B 1 month after the treatment. The excellent, effective and ineffective ef- ficacy rates were 27.0% （20/74 eases） ,41.9% （31/74 cases） and 31.1%（23/74 cases） respectively and the total efficacy rate was 68.9（51/74 cases） in group C 1 month after the treatment. There was statistically significant difference in the efficacy among the three groups. There was no statistically significant difference between group A and B at 3 and 6 months after the treatment, and there was statistically significant difference in the efficacy between group A, group B and group C. Conclusion Percutaneous discectomy combined with ozone injection for lumbar intervertebral disc herniation is a safe and effective method, which is superior to only percutaneous discectomy or only ozone injection.
