食管癌三维适形放疗联合多西它塞同期化疗的临床研究

摘    要:
目的:探讨三维适形放疗(3D-CRT)联合多西它塞同步化疗在食管癌治疗中的近期有效率、毒性反应及生存期。方法:48例病人根据入选标准进入研究。化疗为多西它塞25mg/m2,每周1次。放疗第1天开始应用,采用三维适形放疗,95%PTV59.4Gy/33次/6.5周。结果:完全缓解率为72.92%;近期总有效率(CR PR)为91.67%。1年及3年局部控制率分别为89%、65%,1年及3年生存率分别为85.3%、60.7%。放化组毒性反应主要是放射性食管炎,其次是血液学毒性,患者均能耐受。结论:三维适形放疗联合多西它塞化疗对食管癌近期疗效较好,虽毒性反应增加但病人可以耐受。

Abstract:
Objective:To evaluate the effect and toxicity of three dimensional conformal radiotherapy(3D-CRT)concurrent taxotere for patients with esophageal carcinoma.Methods:Forty-eight patients with esophageal carcinoma were received 3D-CRT concurrent taxotere,the radiotherapy was carried out by 3D-CRT,to a total dose 95%PTV 59.4Gy/33Fr/6.5W.Results:Complete response rate(CR)was 72.92%.Overall response rate(CR PR)was 91.67%.The local control rates of 1 year and 3year were 89% and 65%,respectively.The survival rates of 1 year and 3 year were 85.3% and 60.7%,respectively.The main acute toxic effect was radioactive esophagitis.Conclusion:Three dimension conformal radiotherapy(3D-CRT)concurrent taxotere,could improve the overall response rate and survival rate of esophageal carcinoma,but it increase the acute toxic effct.
