彩色多普勒超声在糖尿病患者第一跖背动脉血流动力学研究中的应用

摘    要:
目的 探讨彩色多普勒超声在糖尿病患者第一跖背动脉血流动力学研究中的应用价值.方法 彩色多普勒超声检测糖尿病患者98只足第一跖背动脉,将其分为2组(A组:糖尿病未并发糖尿病足,n=80;B组:糖尿病并发糖尿病足,n=18),并与正常足(C组,n=64)进行对照,观察第一跖背动脉的来源、走行、血流通畅及充盈情况,分析血管内径、内膜厚度、血流频谱(包括频谱形态、峰值流速、阻力指数).结果 血管内径:B组低于C组(P〈0.01),A、C组无明显差异(P〉0.05);内膜厚度:3间组无明显差异(P〉0.05);峰值流速及阻力指数:A、B组均明显高于C组(P〈0.05),A组低于B组(P〈0.05).结论 彩色多普勒超声对第一跖背动脉血流动力学的检测能为糖尿病足的早预防、早发现、早治疗提供更有价值的客观依据.

Abstract:
Objective To explore the application value of color Doppler ultrasonography in the hemodynamic research on the first dorsal metatarsal artery in patients with diabetes. Methods Color Doppler uhrasonography was applied to detect the first dorsal metatarsal arteries in 98 feet of diabetic patients, which were divided into two groups : group A, diabetes without diabetic foot, 80 feet; group B, diabetes with diabetic foot, 18 feet. 64 feet of normal subjects served as control ( group C). The origination, trend, smooth of bloodstream, angioplerosis status of the first dorsal metatarsal artery were observed. The internal diameter of blood vessel, intima thickness, and bloodstream spectrum such as peak flow velocity and drag index were analyzed. Results The internal diameter of blood vessel in group B was smaller than in group C (P 〈 0.01 ). No significant difference of the internal diameter of blood vessel was found between group A and group B ( P 〉 0.05 ). No significant differences of intima thickness were observed among the 3 groups ( P 〉 0.05 ). The peak flow velocity and drag index in group A and group B were higher than in group C (P 〈 0.05 ). The peak flow velocity and drag index were lower in group A than in group B (P 〈 0.05 ). Conclusion The color Doppler ultrasonic hemodynamic test on the first dorsal metatarsal artery can provide more valuable objective clinical evidence for early prevention, early detection and early treatment for diabetic foot.
