大剂量甲强龙对兔髋关节周围骨组织BMP-2表达的影响

摘    要:
目的:研究大剂量甲强龙对兔髋臼、股骨头、股骨近端骨组织病理表现及骨形态发生蛋白-2(BMP-2)表达的影响。方法:健康新西兰成年大白兔27只,随机分为3组,每组9只。对照组臀部肌内注射3mL生理盐水,1次/d。观察A、观察B组臀部肌内注射甲强龙80mg/kg,1次/d,分别连用3和6d。注射后第2、4、8周的第1天,每组均取3只兔处死,于双侧髋臼、股骨头、小转子高度水平切取约3mm厚的股骨近端骨标本,HE染色观察病理变化,计数空骨陷窝,SABC免疫组化染色观察BMP-2表达情况。结果:3组不同时间点髋臼部位空骨陷窝计数差异无统计学意义(F组间=2.453,F时间=0.008,P均>0.05);但股骨头和股骨近端骨组织空骨陷窝计数差异有统计学意义(F组间=116.782和110.112,F时间=57.207和61.092,P均<0.001),观察组空骨陷窝计数较对照组增加,观察B组增加更明显(P<0.05)。3组不同时间点髋臼、股骨头及股骨近端骨组织BMP-2的表达差异均有统计学意义(F组间=232.623,239.894和338.533;F时间=273.064,198.320和230.326;P均<0.001),观察组BMP-2的表达较对照组降低,观察B组降低更明显(P<0.05)。结论:大剂量甲强龙可以造成髋关节周围骨组织内BMP-2表达降低,严重影响股骨头的骨修复能力。

Abstract:
Aim:To study the effects of high dose methylprednisolone on expression of BMP-2 in acetabulum,caput ossis femoris and proximal femurs bone of rabbit.Methods:A total of 27 adult rabbits were randomly allocated into three groups,9 in each group.The rabbits in group A and group B were injected with methylprednisolone sodium succinate at 80 mg/(kg·d) for 3 and 6 days,respectively.The rabbits in group C were injected with normal saline.At 2,4,and 8 weeks after steroid administration,the bone tissue of acetabulum,caput ossis femoris and proximal femurs were collected to observe histopathological changes,and examine the expression of BMP-2 by immunohistochemical method.Results:The number of bone empty lacunae in caput ossis femoris and proximal femurs in group A and B were more than that of group C(Fgroup=116.782 and 110.112,Ftime=57.207 and 61.092,P<0.001),especially in group B(P<0.05).The average optical density of BMP-2 in acetabulum,caput ossis femoris and proximal femurs of group A and B were higher than those of group C(Fgroup=232.623,239.894,and 338.533;Ftime=273.064,198.320,and 230.326;P<0.001),especially in group B(P<0.05).Conclusion:High dose methylprednisolone can reduce the expression of BMP-2 in bone around hip.
