急性冠脉综合征患者冠脉基质金属蛋白酶-9、妊娠相关血浆蛋白A检测及意义

摘    要:目的 探讨外周静脉及冠状动脉血清基质金属蛋白酶-9(MMP-9)、妊娠相关血浆蛋白A(PAPP-A)水平对冠状动脉粥样硬化斑块稳定性的作用.方法 选取150例冠心病(CHD)患者为试验组,其中急性心肌梗死(AMI)40例、不稳定型心绞痛(UAP)60例、稳定型心绞痛(SAP)50例,另设健康对照组20名.分别测定外周静脉及冠状动脉血清MMP-9、PAPP-A水平,比较各组的差异及其在CHD组中的相互关系.结果 UAP组和AMI组的MMP-9、PAPP-A水平显著高于对照组(P<0.01),亦显著高于SAP组(P<0.05~<0.01).CHD组MMP-9与PAPP-A水平显著相关(P<0.05).结论 MMP-9、PAPP-A水平与急性冠脉综合征(ACS)发病密切相关,测定其水平有助于ACS的早期防治.

Significance and detection of matrix metalloprotetnase-9,pregnancy-associated plasma protein-A in coronary circulation of patients with acute coronary syndrome

Abstract:Objective To investigate the effect of matrix metalloproteinase-9 (MMP-9) and pregnancyassociated plasma protein-A (PAPP-A) on the stability of coronary atherosclerotic plaque. Methods A total of 150 patients with coronary heart disease (CHD) were enrolled as experiment group, in which 40 patients with acute myocardial infarction (AMI) and 60 patients with unstable angina (UAP) as well as 50 patients with stable angina (SAP) were studied and 20 health volunteers served as control. The levels of MMP-9 and PAPP-A were detected and compared. Results The levels of MMP-9 and PAPP-A in experiment group were significantly higher than those of control group (P〈0.01), SAP group (P〈0.05 and 〈0.01 ). There were a positive relationship between MMP-9 and PAPP-A (P〈0.05). Conclusion The levels of MMP-9 and PAPP-A are strongly associated with acute coronary syndrome(ACS). They may be applied in the early prevention and management of ACS.
