成人胆总管囊肿手术的治疗分析

目的：总结成人胆总管囊肿手术治疗的经验体会。方法：对1999年9月—2009年9月收治的32例成人胆总管囊肿患者的临床资料进行回顾分析。结果：术后发生胆漏4例,均经保守治疗痊愈,吻合口狭窄2例。无胰管损伤、胰瘘发生和死亡病例。所有病例均获得随访,未发现癌变病例。手术疗效评价：优26例（81.3%）,良5例（15.6%）,差1例（3.1%）。结论：成人胆总管囊肿患者,接受囊肿切除,肝总管空肠Roux-Y式吻合术可避免术后逆行感染和癌变可能,且操作简单、防反流效果好、术后并发症少,是治疗胆总管囊肿的首选手术方式,值得推广。

Clinical Experience of Surgical Treatment for Adult Cholangiectasis

Objective：To summarize the clinical experience of surgical treatment of adult congenitcal cholangiectasis.Methods： Clinical data of 32 patients with congenital cholangiectasis from September 1999 to September 2009 were collected and analyzed retrospectively.Results： All 32 cases were taken surgical operation,bile duct was damaged（4 cases）,they were recoverd without surgical retreatment,stricture of anastomotic stoma（2 cases）,no case occurred pancreas leak and deaths case.All cases were followed up without occurance of cancer.Follow-up results were excellent in 26 cases（81.3%）,good in 5 cases（15.6%） and poor in 1 case（3.1%）.Conclusions： The choledochal cyst ectomy with Toux-en-Y anastomosis is simple,good anti-reflux and less postoperative complications,therefore it should be the first method of choice for treating adult cholangiectasis.
