心肌肌钙蛋白T检测在婴幼儿肺炎并发心力衰竭中的临床研究

目的：探讨心肌肌钙蛋白T(cTnT)检测在婴幼儿肺炎并发心衰中的意义。同时为婴幼儿肺炎是否并发心衰提供实验依据。方法：对42例肺炎并发心衰及15例同期肺炎而无并发心衰患儿进行cTnT、CK-MB检测。结果：无先天性心脏病肺炎并发心衰息儿，cTnT阳性率不高，射血分数、心轴缩短率及心脏指数测定值也不低；而有先天性心脏病并发肺炎心衰患儿则相反。结论：cTnT检测可作为评价心衰的一项生化指标；婴幼儿肺炎若无先天性心脏病，极少引起心衰，而有心衰者常是发生在有先天性心脏病的基础上。

The Clinical Study of cTnT Detection in Infant Pneumonia Complicated with Heart Failure

Objective:To study the effect of cTnT detection on infant pneumonia complicated with heart failure,as well as to provide experimental evidence to differ whether infant pneumonia with or without heart failure.Methods:cTnT and CK-MB plasma levels,and cardiac function parameters were measured in 42 cases of infant pneumonia with heart failure and 15 cases without heart failure.Results:The positive rate of cTnT was not high in infant pneumonia complicated with heart failure but without congenital heart disease,as well as ejection fraction,fraction shortening and cardiac index were not low.But in the cases with congenital heart disease,the results were reversed.Conclusion:cTnT detection may be a good way to evaluate the level of heart failure.Infant having pneumonia seldom result to heart failure except to complicate with congenital heart disease.
