机械通气联合药物治疗重症支气管哮喘50例临床疗效分析

目的观察机械通气联合药物治疗重症支气管哮喘的临床疗效。方法将100例患者随机分为两组。两组患者入院后均给予糖皮质激素、氨茶碱、B受体激动剂、祛痰剂、抗感染等对症治疗。治疗组同时使用无创呼吸机治疗。观察两组临床效效及症状、体征、肺功能的改善情况。结果疗组临床控制率为78.00%,总有效率为92.00%;对照组临床控制率为58.00%,总有效率为76.00%。临床控制率、总有效率两组相比差异有统计学意义（P〈0.05）。R5、R20、X5、Fres两组治疗前后组内比较差异有统计学意义（P〈0.05）;治疗后两组组间比较差异有统计学意义（P〈0.05）。2组经治疗后日间症状评分、夜间症状评分、PEF占预计值的百分比较治疗前有显著的变化（P〈0.05）。治疗后治疗组咳嗽、气促、肺部干湿性啰音等症状,体征的消失时间,住院天数明显短与对照组（P〈0.05）。结论机械通气联合药物治疗重症支气管哮喘疗效可靠。

Mechanical ventilation combined drug therapy of severe bronchial asthma clinical analysis of 50 cases

Objective To observe the mechanical ventilation therapy of severe bronchial asthma therapy.Methods 100 patients were randomly divided into 2 groups.2 patients were given after admission glucocorticoid,aminophylline,B receptor agonist,expectorant agent,anti-infection and symptomatic treatment.Treatment group,while non-invasive ventilation in the treatment used.2 The clinical effects observed effects and symptoms,signs,improvements in lung function.Results The treatment of the clinical control rate was 78.00%,total effective rate was 92.00%;control group,the control rate was 58.00%,total effective rate was 76.00%.Clinical control rate,the total effective rate between 2 groups was statistically significant compared（P0.05）.R5、R20、X5、Fres 2 before and after treatment group difference was statistically significant（P0.05）;2 groups after treatment between the two groups were significantly（P0.05）.Day 2 After treatment,symptom scores,nighttime symptom score,PEF percentage of total estimated value was significantly higher than before the treatment change（P0.05）.After treatment,cough,shortness of breath,pulmonary symptoms such as dry sex rales,signs of the disappearance time,hospital stay was shorter and the control group（P0.05）.Conclusion Mechanical ventilation combined drug therapy of severe bronchial asthma and reliable.
