腹腔镜下子宫肌瘤剔除术围术期血清应激激素及其他敏感应激指标的变化研究

目的 探讨腹腔镜下子宫肌瘤剔除术围术期血清应激激素及其他敏感应激指标的变化。方法 选择2012年12月~2014年1月来本院就诊的54例需要剔除子宫肌瘤的患者为研究对象,将其随机分为对照组和观察组,每组各27例,对照组患者采用开腹子宫肌瘤剔除术治疗,观察组患者采用腹腔镜子宫肌瘤剔除术治疗,观察比较术前10 min、术中及术后30 min血清中应激激素和其他应激激素的变化。结果 观察组患者的平均手术时间、平均术中出血量与对照组比较,差异无统计学意义（P〉0.05）;观察组患者的平均住院时间明显短于对照组,差异有统计学意义（P〈0.05）。观察组患者术后30 min的NE、Cor与对照组比较,差异无统计学意义（P〉0.05）。观察组患者AngⅡ的变化幅度与对照组比较,差异有统计学意义（P〈0.05）。观察组患者术后30 min的5-HT、Lep和血糖浓度均显著降低,与对照组比较差异有统计学意义（P〈0.05）。结论 腹腔镜子宫肌瘤剔除术患者围术期机体应激激素变化小,更有利于手术的进行和患者术后的康复。

Change research of perioperative serum levels of stress hormones and other sensitive stress index in hysteromyomectomy under laparoscope

Objective To investigate the change of perioperative serum levels of stress hormones and other sensitive stress index in hysteromyomectomy under laparoscope. Methods 54 patients treated for myomectomy in our hospital from December 2012 to January 2014 were selected as research objects,they were randomly divided into control and observation group,each group had 27 cases.The control group was given open myomectomy treatment and the observa- tion group were treated by laparoscopic myomectormy,then the changes of stress indexes of patients with laparoscopic myomectomy in preoperative 10 min,intraoperative and postoperative 30 min were compared. Results The average oper- ation time and average bleeding during the operation in observation group compared with the control,there was no sta- tistical significance （P〉0.05）,the average hospitalization time in the observation group was significantly less than that of the control group,and the difference was statistically significant （P〈0.05）.The NE and Cor postoperative 30 min in ob- servation group compared with the control,there was no statistical significance （P〉0.05）,but the degree of varibility of Ang I1 in the observation group was siginificantly lower than that of the control group,there was statistically significance （P〈0.05）.The 5-HT, Lep and blood glucose concentrations of postoperation 30 min in the observation group were significantly lower,there was statistically significance compared with the control group （P〈0.05）. Conclusion Laparoscopic uterine fibroids excluded patients perioperative body＇s stress hormones change is small,it is more conducive to the operation and patients postoperative rehabilitation.
