PBF多普勒定量分析在慢性病毒性肝炎与炎症分级中的应用

目的探讨彩色多普勒超声在慢性病毒性肝炎的静脉血流多普勒定量与肝组织炎症分级的相关性。方法本文对120例慢性病毒性肝炎患者及50例正常人的门静脉系统进行多普勒超声检查，测量门静脉主干内径、血流速度、血流量、凝血指数的血流动力学改变，并与肝组织炎症分级进行分析对比，探讨其相关性。结果门静脉主干直径（PD）及淤血指数（CI）与炎症坏死程度（G）呈显著正相关关系（0．55·0．56）。门静脉最大流速（PVx）及平均流速（PVm）则呈显著负相关关系（-0．56·-0．43），而门静脉血流量（PBF）无明显相关关系（-0．19）。结论门静脉主干，门静脉最大流速，门静脉平均流速及淤血指数可用于评价慢性病毒性肝炎炎症坏死活动度，特别是在鉴别中重度（G3—4）与轻度（G1—2）炎症；正常与G2级病变等方面较重要意义。

PBF quantiative analysis using colored Dopplor for clinical phasing of chronic hepatitis and hepatic tissue inflamation

Objective To discuss the correlation of quantitative analysis of portal venous blood flow and phasing of liver tissue inflamation in chronic hepatitis B patients by color Dopplor.Methods There 120 chronic hepatitis B patientts and 50 healthy subjects were included in this observation.The inner diameter of portal vein,the velocity of blood flow,coagulation index and hemodynamic changes were determined and compared with hepatic tissue inflamation phasing.Results The portal diameter and coagulation index were positively correlated with severiity of hepatic tissue inflamation(r=0.55,0.56).The maximum venous blood flow was negatively correlated with average venous blood flow(r=-0.56,-0.43).While there no correlation was observed between portal venous blood flow and hepatic tissue infalmation.Conclusion The portal vein,indexes of maximum and average venous blood flow can be used for assessment of severity of heapatic tissue inflamation.
