小剂量胺碘酮治疗充血性心力衰竭并室性心律失常疗效观察

目的探讨小剂量胺碘酮治疗充血性心力衰竭并室性心律失常的临床疗效及安全性。方法选择充血性心力衰竭并室性心律失常患者84例,按入院先后顺序随机分为观察组与对照组,每组42例。对照组常规给予抗心衰治疗,观察组在对照组治疗的基础上给予口服胺碘酮片治疗,第1周200mg,3次/d;第2周服药频率改为2次/d;第3周及以后改为1次/d,规律服药8周,用药期间根据具体情况适当调整剂量。对两组的总有效率、左心室射血分数及不良反应发生率等临床指标进行观察比较。结果治疗后,观察组的有效率及左心室射血分数均高于对照组,组间差异具有统计学意义（P〈0.05）;不良反应发生率观察组略高于对照组,但组间差异不具有统计学意义（P〉0.05）。结论在常规充血性心衰治疗的基础上,小剂量胺碘酮对充血性心衰合并室性心律失常患者具有良好的临床疗效,且安全,可在临床推广应用。

Low-dose amiodarone in treatment of congestive heart failure with ventricular arrhythmias

Objective To explore the clinical efficacy and safety of low-dose amiodarone in treatment of congestive heart failure with ventricular arrhythmias.Methods 84 patients with congestive heart failure by ventricular arrhythmias were included in the study.According to admission order,they were randomly divided into observation group and control group(n=42).Control group were given conventional anti-heart failure treatment.In the control group,based on the observation group were treated with oral amiodarone in treatment,1 week 200mg,tid;frequency of medication 2 weeks to bid,the first 3 weeks and later changed to qd,regular medication for 8 weeks during treatment appropriate dose adjustment according to specific circumstances.All the clinical indicators of the two groups were observed and compared including efficiency,left ventricular ejection fraction and the incidence of adverse reactions.Results For the incidence of adverse reactions,the observation group was slightly higher,but the difference was not statistically significant between the groups(P>0.05).Conclusion In the routine treatment of congestive heart failure,low-dose amiodarone have good clinical efficacy and safety of a good by nature in treatment of congestive heart failure with ventricular arrhythmias,is worthy of clinical application.
