游离腹内斜肌-髂骨骨肌瓣修复双侧上颌骨术后缺损

目的评价游离髂骨骨肌瓣修复双侧上颌骨术后缺损的效果。方法采用带腹内斜肌的游离髂骨骨肌瓣修复双侧上颌骨术后缺损2例。游离髂骨塑形后重建上颌骨弓,腹内斜肌分隔口鼻腔或替代硬腭黏膜,并将供血的旋髂深动静脉血管与面动静脉血管吻合。结果2例游离髂骨骨肌瓣均存活,无并发症发生。腹内斜肌肌肉修复硬腭黏膜厚度适中,能够很快上皮化。经口进食,发音及外形满意。结论游离髂骨肌瓣修复双侧上颌骨术后缺损是一种理想的手术方法。

Reconstruction after Bilateral Maxillectomy Using Deep Circumflex Iliac Artery Free Flap with Internal Oblique Abdominal Muscle

To evaluate the reconstruction after bilateral maxillectomy with deep circumflex iliac artery(DCIA)free flap with internal oblique abdominal muscle.Methods The DCIA free flap with internal oblique muscle was used to restore the defect after bilateral maxillectomy in two patients.The bone was reshaped to recreate the maxillary arch.The internal oblique abdominal muscle was inserted to close the oro-nasal fistula,or replaced the palate mucosa.The pedicle of flap was anastomosed to facial artery and vein.Results Both of the osteomuscular flaps survived.There was no surgical complications.The muscle epithelialized rapidly, providing an excellent oral appearance with minimal bulk.The appearance,speaking and swallowing functions were good.Conclusions This flap is one of the excellent resort for reconstruction of bilateral maxillectomy detect.
