髂内动脉超选择性栓塞化疗术治疗晚期前列腺癌

目的 研究髂内动脉栓塞术在晚期前列腺癌治疗中的作用.方法 60例晚期前列腺癌患者随机分为两组,介入组28例,行双侧髂内动脉超选择性栓塞化疗并行双侧睾丸切除术加内分泌治疗;对照组32例,行双侧睾丸切除术加内分泌治疗.对两组治疗后的前列腺体积变化情况、尿流率及前列腺特异性抗原（PSA）等进行比较.结果 治疗后2周介入组前列腺体积较对照组有明显的缩小（P＜0.05）,尿流率改善显著（P＜0.05）,PSA降低更明显（P＜0.05）.介入组与对照组3年生存率分别为70.59%、66.67%（P＞0.05）.结论 前列腺癌的髂内动脉超选择性栓塞化疗术是治疗晚期前列腺癌有效的方法之一,尤其是对雄激素非依赖性的前列腺癌具有更大的意义.

Therapeutic effect of super-selective embolism and chemotherapy in internal iliac artery on advanced prostatic carcinoma

Objective To observe the therapeutic effect of super-selective embolism and chemotherapy in internal iliac artery on advanced prostatic carcinoma.Methods 60 patients with advanced prostatic carcinoma were randomly divided into two groups,intervention group (n=28) and control group (n=32).28 patients in intervention group were treated by super-selective embolism and chemotherapy in internal iliac artery,surgical castration and endocrinotherapy.32 patients in control group were treated by surgical castration and endocrinotherapy.Results Two weeks after treatment,the size of prostates in intervention group was significantly smaller than that of control group (P<0.05);the uroflow rate was significantly improved,as compared with that of control group (P<0.05);PSA was reduced significantly,as compared with that of control group (P<0.05).The 3-year survival rates were 70.59%,66.67% respectively (P<0.05).Conclusion The super-selective embolism and chemotherapy in internal iliac artery is effective in treating advanced prostatic carcinoma,especially for the prostate carcinoma that doesn't depend on male hormone.
