检测补体C3、C4及C1q水平对狼疮性肾炎活动性判定的价值

目的：检测活动期和非活动期狼疮性肾炎（LN）患者血清补体C3、C4及C1q水平,探讨其对LN活动性判定的价值.方法：用免疫比浊法分别对经临床确诊的82例活动期和79例非活动期LN患者血清补体C3、C4及C1q的水平进行检测,比较两组患者C3、C4及C1q的水平及阳性检出率.结果：两组患者血清补体C3和C4的阳性检出率差异无统计学意义（P＞0.05）,LN活动组C1q的阳性检出率（92.6％）高于LN非活动组（70.8％）,差异有统计学意义（P＜0.05）;LN活动组补体C3、C4、C1q水平均较LN非活动组下降（P＜0.05）.结论：LN活动期患者C1q阳性率较LN非活动期患者高,而C3、C4、C1q水平较LN非活动期患者均降低,动态监测C3、C4、C1q的水平及检测C1q阳性情况对判断LN是否处于活动期有重要意义.

The Value of Detecting Complement C3, C4 and C1q Levels in Deciding of Lupus Nephritis Activity

Objective： To detect the serum levels of complement C3, C4 and Clq of patients with lu- pus nephritis （LN） in active period and inactive period, and so as to evaluate the value of them in deciding of LN activity. Methods： Immunoturbidimetry method was employed to detect serum levels of C3, C4 and Clq of 82 LN patients in active period and 79 LN patients in inactive period. Serum C3, C4 and Clq levels and their positive detection rates of the two groups were compared. Results： No sig- nificant difference was found in positive rates of C3 and C4 between the two groups （ P 〉 0.05 ）, while positive rate of Clq in LN active group （92.6%） was significant higher than that of LN inactive group （70.8%） （P 〈0.05）. Serum levels of C3, C4 and Clq in LN active group were lower than those of LN inactive group（ P 〈 0.05 ）. Conclusions： Positive rate of Clq in LN active group is significant higher than that of LN inactive group, while serum levels of C3, C4 and Clq in LN active group are lower than those of LN inactive group. Dynamic monitor of C3, C4 and Clq levels and Clq positive rate has important meaning in deciding of LN activity.
