16层螺旋CT在肺动脉栓塞诊断中的应用

摘    要:目的 研究16层螺旋CT在肺动脉栓塞诊断中的应用价值.方法 使用GE LightSpeed16 16层螺旋CT机对12例肺栓塞病人行造影增强扫描,将原始数据进行重建再处理.结果 共分析12例病人738支肺动脉,其中193支肺动脉分支显示栓塞,占26.2%.直接征象为充盈缺损,包括:中心型充盈缺损30支,偏心型充盈缺损102支,附壁血栓16支,完全闭塞15支;间接征象包括:马赛克征1例,肺梗死3例,胸腔积液4例,肺动脉高压2例.结论 16层螺旋CT肺动脉造影方便、快捷、无创,可以作为诊断肺栓塞的首选方法.

Abstract:Aim To study the role of 16-detector row spiral CT in the diagnosis of pulmonary embolism.Methods Twelve patients with pulmonary embolism were examined with 16-detector row spiral CT(GE LightSpeed16)pulmonary angiography.Then the raw data were reformed and reprocessed.Results In 12 patients studied,738 branches of pulmonary arteries were analysed,and among them 193 branches were involved with pulmonary embolism(26.2%).The direct sign was the filling defect in the pulmonary arteries branches,including central filling defect in 30 branches,eccentric filling defect in 102 branches,embolism attached to the wall of host artery in 16 branches,total occlusion of the pulmonary artery in 15 branches.The indirect signs included mosaic sign(1 case),pulmonary infarction(3 cases),pleural effusion(4 cases),pulmonary hypertension(2 cases).Conclusion The 16-detector row spiral CT pulmonary angiography is a convenient,fast,and non-invasive method for pulmonary embolism,and it may be the preferable method for the diagnosis of pulmonary embolism.
