前循环动脉瘤手术时机与预后分析

目的探讨前循环动脉瘤手术时机及对预后的影响。方法回顾性分析34例前循环动脉瘤显微手术,早期手术（72h内手术）24例,延期手术（72h～2周内手术）10例,术式采用Yasargil翼点入路,其中瘤颈夹闭术33例,瘤壁加固术1例。结果早期手术患者24例,恢复良好22例,轻残1例,重残1例,好转率92%;延期手术患者10例,恢复良好6例,轻残2例,重残1例,死亡1例,好转率60%。结论早期手术是避免动脉瘤再破裂出血,提高治愈率和降低死亡率及致残率的关键。

Operative Timing and Prognostic Analysis of Front Circulation Aneurysm

Objective Study of effect on operative timing and prognostic analysis of front circulation aneurysm.Methods Analysis of 34 cases of front circulation aneurysm:24 cases of early operation(within 72 h),10 cases of delayed operation(between 72 h to 14 d).Operation type was used by the Yasargil pterion approach,in which 33 cases were clipped the aneurysm neck and 1 cases was reinforced the aneurysm wall.Results 22 cases were good recovery,1 case was mild disability and 1 case was major disability in total 24 cases.Improvement rate was 92%.6 cases were good recovery,2 cases were mild disability,1 case was major disability and 1 case was death in total 10 cases.Improvement rate was 60%.Conclusion Early operation is a key to avoiding aneurysm rupture and bleeding,improving the cure rate and reducing the mortality.
