非瓣膜性心房颤动并脑卒中发病及抗栓用药的分析

目的观察非瓣膜性房颤脑卒中的发生率及比较各抗栓用药组随访1年脑卒中的发生率。方法 1998年1月-2009年1月非瓣膜性房颤病例388例,比较各抗栓用药组及未用药组随访1年脑卒中的发生率。结果 388例非瓣膜性房颤脑卒中的发生率约为7.2%。各用药组及未用药组随访1年后脑卒中的发生率的差异有统计学意义（P=0.043）。结论非瓣膜性房颤脑卒中的发生率较高,已成为老年人致残、致死的重要原因。华法令、阿司匹林较大剂量（150mg及以上）在抗栓治疗效果上较阿司匹林75mg及未用药组差异有统计学意义。

Study on the Incidence of Stoke with Nonvalvular Atrial Fibrillation and the Optimal Anticoagulation Therapy

Objective The aim of the study was to identify the incidence of nonvalvular atrial fibrillation with stoke and the optimal anticoagulation therapy to prevent stoke with nonvalvular atrial fibrillation.Methods A total of 388 adult patients diagnosed with nonvalvular atrial fibrillation came from the admitted patients of our cardiopathy department from 1998 to 2009 were compared the incidence of stoke after receiving different anticoagulation therapy group for 1 year.Results The patients with the nonvalvular atrial fibrillation had a about 7.2% incidence of stoke.The incidence of stoke in any anticoagulation therapy was significantly lower than that of the control（P=0.043）.Conclusion The stoke had a high incidence among the patients with the nonvalvular atrial fibrillation and had become the main cause of mutilation and death in old mans.The warfarin and aspirin（150 mg and more） can more significantly decrease the incidence of stroke than the aspirin（75 mg） and control.
