地佐辛和芬太尼联合应用在骨科手术后自控静脉镇痛的疗效观察

摘    要:目的:比较联合应用地佐辛和芬太尼与单用芬太尼在骨科手术术后镇痛的疗效观察。方法:行择期全麻手术的ASA1~2级患者60例,年龄25~45岁,随机分为两组。术后均使用静脉镇痛泵,2ml/h,A组(地佐辛+芬太尼)镇痛泵配方为:地佐辛0.5mg/kg+芬太尼10μgkg+昂丹司琼4mg+0.9氯化钠注射液至100ml;B组(芬太尼)镇痛痛配方为:芬太尼20μg/kg+昂丹司琼4mg+0.9氯化钠注射液至100ml,分别于术后4、8、12、24、48h采用视觉模拟评分法(VAS)评定镇痛效果和镇静评分标准(SS)及不良反应。结果:两组镇痛效果及不良反应均有显著性差异。结论:联合应用地佐辛和芬太尼在骨科手术术后镇痛的效果优于单用芬太尼的效果。

Abstract:Objective:o compare the analgesic effects of Fentanyl with Fentanyl combined Dezocine.Methods:60 patients(ASA1~2,25~45 years) to received general anesthesia in orthopedics selective surgery was divided into two groups.They received patient-controlled intravenous analgesia(PCIA,2 ml /h) after operation.The(D+F) group PCIA solution contained Dezocine(0.5mg/ kg),Fentanyl(10μg/kg),Ondansetron(4mg) and 0.9% Nacl solution;The F group contained Fentanyl(20μg/mg),Ondansetron(4mg) and 0.9% Nacl solution.VAS(0=no pain.,10=worst pain) and(SS)was used to measure pain intensity,sedateness and their side effects were recorded at 4,8,12,24h after operation.Results:There was significant difference in VAS score and side effects between the two groups.Conclusion:Dezocine combined Fentanyl may obtain superior analgesic effect and less side effects compare with Fentanyl after orthopedics operation.
