后外侧小切口在普胸外科手术中的应用

摘    要:目的 普胸外科手术常用的后外侧标准切口 ,切断肌肉多 ,需切除或切断肋骨 ,术后胸痛较重 ,肩关节功能障碍发生率高等缺点。针对上述情况 ,我们设计了后外侧小切口。方法 以肩胛下角外下方 2~ 3cm处为起点、向前下至腋中线或腋前线为止点 ,长 14~ 18cm。结果  1996年 1月至今应用于食管癌、肺癌根治性切除 ,肺良性肿瘤及纵隔内支气管囊肿切除等普胸外科手术 32 0例 ,均顺利完成手术。结论 该切口较标准后外侧切口优点为避免了切断大片的胸壁肌肉 ;不需切除肋骨 ;减少了出血量 ;节省了开、关胸的时间 ;减轻了术后胸痛的症状 ,降低了肩关节功能障碍的发生率 ;胸腔感染率降低 ;减轻了病人术前恐惧感及术后精神压力。值得临床推广应用

Abstract:Objective Generally speaking, incision of general chest surgery operations often has many flaws, such as more muscle cuts,ribs cuts and this method usually cause chest pain and disablility of shoulder joint. In order to solve this problem, the posterior lateral short incision was devised. Method The incision is from the beginning point 2 3cm outside and beneath the foreland below scapula down to anterior axillary line or midaxillary line. The length was 14 18 cm. Results General chest operations were performed successfully in 320 cases from 1996 till now, including radical resections of esophageal carcinoma?lung cancer and mediastinal tumor resection ,etc. Conclusion All these operations have been accomplished method characteristic of such merits as avoid cutting more muscle, avoiding cutting off ribs; lessening hemorrhage, shortening operation time; reducing complications such as disablity of scapula joint and chestpain,and less ening preoperative fear and postoperative press, etc.
