应用单侧多功能外固定支架治疗Ⅲ型胫骨开放骨折的体会

探讨单侧多功能外固定支架在治疗Ⅲ型胜骨开放骨折中的价值及操作技巧。方 法床用单侧多功能外固定支架治疗胫骨开放骨折２２例，操作中粉碎骨块尽可能用螺钉初步固 定；在不使钢针进入血肿的前提下，缩短固定杆内套管，使近骨折处的两枚针尽可能摇近骨折处； 强调最外侧两枚钢针向骨折端成５°角放置，在针尾施以预应力使与其它钢针平行后置于针夹内， 以增加固定强度。结果：２２例中２０例获骨愈合（愈合率９０．９％）。结论：通过对操作技术的改进， 单侧多功能外固定支架具有较强的固定钢度，可显著提高Ⅲ型胫骨开放骨折的愈合率。

Application of Unilateral Multifunctional External Fixation Brace for the Treatment of Type III Open Tibial Fracture

Objective: To discuss the value and operative technique of the unilateral multifunctional external fixation brace for the treatment of type III open tibial fracture. Method: Twenty-two cases of type III open tibial fracture were treated with this unilateral multifunctional external brace. Comminuted fragments were assembled with screw firstly. Two needles were placed proximally to the fracture end as possible with the hemorrhage zone being not intervened by the needles. The another two distal needles were placed at an angle of 50 to the fracture end and then it was forced into the clip under stress so as to increase the fixation strength of the brace. Results: Bone union was obtained in 20 of the 22 cases, with an union rate of 90. 9% . Conclusion: With this technique, the fixation strength of the unilateral multifunctional brace was improved and bone union rate increased obviously.
