摘 要: | OCT影像成像技术适用于可弯曲支气管镜检查,它在肺部病变诊断学上具有重要的作用和潜力。目前流行的诊断性影像学在探测人支气管的气道形态早期的疾病形态变化和实时区别肺部疾病的非特异性的病理学变异,不具有充分的分辨率和组织穿透力能力。OCT具有的空间分辨率再现了支气管内的剖面、外形,能获得高分辨率、横断层面组织学的缩微图象,可以认为其光学的活组织检查能够取代常规的切除活组织检查。它是一种高度可行的光学工具,实时检测并获得接近组织学成像的支气管内的病理学,在肺部疾病的监测、(鉴别)诊断和治疗是很有潜力的。OCT现在的应用策略是内窥镜检查对病变的早期诊断。光学的活组织检查通过使用OCT 系统追踪、观察和评估治疗的效果。

Abstract: | Optical coherence tomography (OCT) is an imaging technology that may be adapted for use with flexible fiberoptic bronchoscopy, potentially allowing it to play an important role in pulmonary diagnostics. Current diagnostic imaging modalities for human bronchial airways do not possess sufficient resolution and tissue penetration depth to detect early morphologic changes and to differentiate in real-time pulmonary disease pathology from nonspecific aberrations. OCT possesses the requisite high spatial resolution for reproducible delineation of endobronchial wall profiling. OCT can obtain high-resolution, cross-sectional microimages of tissue potentially allowing for optical biopsy to substitute for conventional excisional biopsy.It is a highly feasible optical tool for real-time near-histologic imaging of endobronchial pathology with potential for pulmonary disease surveillance applications in diagnosis and treatment. OCT should revamp the present strategies for endoscopic early diagnosis. Optical biopsy using by OCT system is considered to be useful in follow up to dysplasia as well as evaluation of therapeutic outcome. |
