肩胛骨X线摄片对肩峰下撞击综合征的诊断价值

目的:探讨肩胛骨正侧位X线摄片对肩峰下撞击综合征患者的诊断价值。方法:选择肩峰下撞击综合征50例患者的58肩进行肩胛骨正侧位X线拍片,43例49肩同时行利多卡因试验,分析X线结果,比较三型利多卡因试验阳性率。结果:Ⅰ型肩峰13.8%(8/58),Ⅱ型肩峰25.9%(15/58),Ⅲ型肩峰60.3%(35/58);Ⅰ型肩峰中7肩行利多卡因试验,阳性率为42.9%(3/7),Ⅱ型肩峰中13肩行利多卡因试验,阳性率53.8%(7/13),Ⅲ型肩峰中29例行利多卡因试验,阳性率75.9%(22/29)。经χ2检验,Ⅲ型肩峰组与其他两组的利多卡因试验阳性率比较差异均有显著性(P<0.05),Ⅰ型肩峰组与Ⅱ型肩峰组的利多卡因试验阳性率差异无显著性(P>0.05)。结论:肩胛骨正侧位X线片对诊断肩峰下撞击综合征诊断价值高;Ⅲ型肩峰者罹患肩峰下撞击综合征的可能性较Ⅰ型、Ⅱ型肩峰者明显增高。

The diagnostic value of X-ray film of scapula adem position in Subacromial Impingement Syndrome

Objective:To evaluate the diagnostic value of X-ray film of scapula adem position in patients with Subacromial Impingement Syndrome.Method:There are 50 patients(58 shoulders) who have chief complaint of shoulder painfulness and Impingement test are positive to do the X-ray of scapulas with adem position,and analyze the outcomes.43 patients(49 shoulders) did the lidocaine test and the results of X-ray were analysed.And then compared with another group which did not take lidocaine tests.Result:There are 8 shoulders in type Ⅰ,accounting for 13.8%(8/58),15 shoulders in type Ⅱ,accounting for 25.9%(15/58),35 shoulders in type Ⅲ,accounting for 60.3%(35/58).After the lidocaine test,there are 3 shoulders in type I,positive incidence is 42.8%(3/7),7 in type Ⅱ,positive incidence is 53.8%(7/13),31 in type Ⅲ,positive incidence is 75.9%(22/29).The type Ⅲ has a significant difference(P﹤0.05) to other types in the positive results of lidocaine test through chi-square test,but there was no significant difference between type I and type Ⅱ(P﹥0.05).Conclusion:(1) There are great diagnostic value of X-ray film in Subacromial Impingement Syndrome;(2)Patients of type Ⅲ have a greater possibility of getting Subacromial Impingement Syndrome than type I and type.
