老年下肢骨折病人D-二聚体定量检测的临床价值

目的:探讨老年下肢骨折病人D-二聚体定量检测的临床价值。方法:采用增强型乳胶免疫比浊法对57例老年下肢骨折患者术前、术后第3天及术后第7天进行血浆D-二聚体的定量检测,跟踪观察患者病情。结果:57例患者有7例并发深静脉血栓(DVT),并发DVT组术前、术后第3天及术后第7天血浆D-二聚体的检测结果有显著性差异(P<0.05),呈进行性增高。未并发DVT组术前、术后第3天及术后第7天血浆D二聚体的检测结果无显著性差异(P>0.05)。而并发DVT组与未并发DVT组相比较:术前血浆D-二聚体的检测结果无显著性差异(P>0.05);术后第3天血浆D-二聚体的检测结果有显著性差异(P<0.05);术后第7天血浆D-二聚体的检测结果有显著性差异(P<0.01)。结论:血浆D-二聚体的动态监测对骨折术后并发血栓性疾病具有早期诊断价值。

Clinical evaluation of plasma D-dimer assay in aged patients with lower limbs fracture

Objective To evaluate the measurement of plasma D-dimer level in lower limbs fracture of aged patients. Method Automated latex immunoassay for the determination was used to detect plasma D-dimer in 57 aged patients with lower limbs fracture during the phases preoperative 3 days and postoperative 7 days. Results 7 patients occur deep vein thrombosis(DVT) . The different level of D-dimer during the phases preoperative 3 days and postoperative 7 days in DVT group was significant (P<0.05). The different level of D-dimer during the phases preoperative 3 days and postoperative 7 days in NO DVT group was not significant (P>0.05). The different level of D-dimer between two groups: preoperative surgery was not significant(P>0.05),postoperative 3 days was significant(P<0.05). postoperative 7 days was significant (P<0.01). Conclusion Automated latex immunoassay for the determination of D-dimer is very useful for predicting occurrence of thromboembolism in aged patients with lower limbs fracture.
