双J管在治疗上尿路梗阻中的应用及原理

目的探讨双J管在尿路梗阻术后的应用效果以及原理。方法2001年3月至2005年12月,我院治疗84例上尿路梗阻患者(单纯输尿管结石66例,输尿管狭窄合并结石10例,UPJO者5例,UBJO者3例),术后留置1～2根双J管。结果84例患者共留置102根双J管,术后留置时间3周～2.5年,平均5周。37例患者出现不同程度的下腹胀痛、尿痛、血尿等不适,拔管后症状缓解。15例患者双J管上附着结石形成。2例患者双J管上移,输尿管镜下拔除。2例患者双J管下移末端部分顶住膀胱黏膜,血尿较重,提前拔管后好转。结论治疗上尿路梗阻术后留置双J管具有操作简便、引流效果好、并发症少等优点,可以作为术后常规治疗手段。

Effectiveness and principle of Double-J stent in the treatment of upper urinary obstruction

Objective To explore the effectiveness of Double-J stent in the treatment of upper urinary obstruction and the preparation of the relative complications.Methods From March 2001 to December 2005,84 cases who had upper urinary obstruction(66 cases had ureteral calculi purely,10 cases had ureteral calculi concomitant with ureteral obstruction,5 cases had a UPJO,3 cases had a UBJO)were detained one or two Double-J stent after the operation for primary disease.Results A total of 102 Double-J stents were detained in 84 cases.The mean lasting time of Double-J indewell was 5 weeks,ranged from 3 weeks to 2.5 years.37 cases had a extensive degree of unsatisfaction,including hypogastric pains,odynuria,haematuria and so on.Double-J were moved up in 2 cases,and were removed by ureteroscopy.2 cases had a severe heamtauria due to the Double-J stent moved down to outlet of bladder,and were released after the stent were removed.15 cases had calculi formed on the tip of Double-J in the bladder.Conclusions The placement of Double-J after operation for upper urinary obstruction had a advantage of easiness,good effectiveness in draining,little complication,and should be the routine method for upper urinary obstruction.
