20例产科多器官衰竭临床分析

目的探讨产科多器官功能衰竭的诱因、诊断及防治措施。方法对20例MOF的临床资料进行回顾性分析。结果导致产科MOF的主要因素为重度妊高征、产科出血及产褥感染。常见衰竭器官依次为脑衰、肾衰、呼衰、凝血功能障碍及心衰等。孕产妇抢救成功14例,死亡6例,死亡率30%;围生儿20例,死亡9例,死亡率45%。结论加强孕期管理,积极治疗原发病,早期诊断和治疗,有效控制感染是取得防治成功的前提,保护相关脏器功能的支持疗法及相关科室的配合亦至关重要。

CLINICAL ANALYSIS OF 20 CASES OF MULTIPLE ORGAN FAILURE IN OBSTETRIC DEPARTMENT

Objective To probe into inducement, diagnosis, prevention and treatment of multiple organ failure in obstetric department. Method Retrospective analysis of clinical data of 20 cases of MOF. Result The main factors induc- ing MOF in obstetric department are advanced pregnancy-induced hypertension, obstetric bleeding, and puerperal infec- tions. The function failures frequently appeared in brain, kidney, respiratory, blood coagulation and heart in tern. The ma- ternity patients 14 cases survived, 6 cases died, mortality rate 30%. Perinatal newborn 20 cases, 9 died, mortality rate 45%. Conclusion Enhance the management of pregnancy period, treat the primary diseases actively, diagnosis and treat- ment early, control infections effectively are preconditions to achieve success, and the supporting treatment to protect corre- sponding organ function and cooperate of relevant discipline are also essential.
