腹腔镜下子宫肌瘤剔除术手术时间影响因素分析

目的探讨腹腔镜下子宫肌瘤剔除术中影响手术时间的因素。方法腹腔镜下对70例子宫肌瘤患者行子宫肌瘤剔除术。观察肌瘤数、肌瘤直径、肌瘤部位、术中缝合与否对手术时间、出血、术后血红蛋白下降的影响。结果肌瘤数≥3个、子宫肌瘤位于深肌壁间、术中需要缝合子宫切口关闭残腔患者手术时间均比对应组长，但术中出血、术后血红蛋白下降无显著性差异。子宫肌瘤位于子宫前壁或后壁、子宫肌瘤直径对手术时间、术中出血无明显影响。结论在有比较熟练的腹腔镜下手术技术的基础上，可适当放宽子宫肌瘤剔除术手术指征，虽手术时间相对较长，但对术中出血、术后恢复影响不大。

On Factors Influencing on Operation Time of Rejecting Hysteromyoma under Laparo

Objective The objective is to discuss the factors influencing on operation time of rejecting hysteromyoma under laparo. Methods Seventy patients underwent this operation. The influence of muscular tumor numbers, diameter and position of it and the saturation on time of operation, bleeding and lib decreasing after operation was observed. Results The muscular tumor numbers were equal to or more than three. The hysteromyoma was at inter-walls of the inner muscles. The operation time of sewing up the womb incision to shut the leftover cavity was longer than that of the contrast group. But there was no remarkable difference in aspects of bleeding and lib decreasing after operation. There was no noticeable impact brought by the position or diameter of the hysteromyoma. Conclusion Based on skilled operation technique under laparo, operation indexes of it can be enlarged a bit. Though the time is relatively long, there is no big influence on bleeding during the operation and recovery after it.
