子宫颈病变中Nrf2和Keap1蛋白的表达及与预后的关系

摘    要:目的探讨氧化应激蛋白核因子E2相关因子(nuclear factor E2-related factor,Nrf2)和Kelch样ECH相关蛋白1(Kelch-like ECH-related protein 1,Keap1)在子宫颈病变中的表达及与预后的关系。方法收集20例子宫颈上皮内病变和67例子宫颈癌(Ia期21例、Ib期24例、IIa期22例)标本,采用免疫组化SP法检测Nrf2和Keap1蛋白在子宫颈病变组织中的表达,并分析两者表达与患者预后的关系。结果Nrf2和Keap1蛋白表达定位于子宫颈上皮细胞质,呈浅黄色至棕黄色;随着子宫颈病变病理分级的升高,Nrf2蛋白在子宫颈上皮内病变,子宫颈癌Ia、Ib、IIa期中的阳性率分别为25.0%、61.9%、75.0%、81.9%,Keap1蛋白的阳性率分别为75.0%、66.6%、54.2%、45.5%。Keap1蛋白表达与肿瘤肌层浸润、有无淋巴结转移、分化程度、临床分期有关。结论Nrf2和Keap1蛋白表达可能与子宫颈病变的发生、发展以及预后相关。

Abstract:Purpose To investigate the expression of nuclear factor E2-related factor(Nrf2)and Kelch-like ECH-related protein 1(Keap1)proteins in cervical lesions and relationship with prognosis.Methods 20 cases of cervical epithelial lesions and 67 cases of cervical cancer(21 cases of stageⅠa,24 cases ofⅠb,22 cases ofⅡa)were collected.The Nrf2 and Keap1 proteins expression in cervical tissues was examined by immunohistochemical SP method,and the relationship between their expression and prognosis was analyzed.Results The expressions of Nrf2 and Keap1 were distributed in the cytoplasm of cervical epithelium and were pale yellow to brown-yellow.With the increase of pathological grade of cervical lesions,the positive rate of Nrf2 protein in cervical intraepithelial lesion andⅠa,Ⅰb andⅡa stage were 25.0%,61.9%,75.0%and 81.9%,respectively.The expression rate of Keap1 protein were 75.0%,66.6%,54.2%and 45.5%,respectively.Keap1 expression was related to tumor muscular layer infiltration,lymph node metastasis,degree of differentiation,and clinical stage.Conclusion Oxidative stress proteins Nrf2 and Keap1 may be related to the development and prognosis of cervical lesions.
