促性腺激素释放激素抑制剂在卵巢功能低下患者促排卵中的应用

目的:比较卵巢功能低下患者应用促性腺激素释放激素抑制剂(GnRH antagonist,GnRH-A)促排卵与促性腺激素释放激素激动剂(GnRH agonist,GnRH-a)短方案促排卵有无差异。方法:A组应用GnRH-a与HMG/rFSH(Gn)促排卵共53个周期,B组应用HMG/rFSH(Gn)与GnRH-A促排卵共30个周期,比较两组HCG注射日的血清E2与LH水平及内膜厚度、卵泡刺激素用量及所用天数,获卵数、受精率、卵裂率、种植率和妊娠率。结果:B组与A组相比,两组的促卵泡激素(Gn)所用天数有明显差异(10.87±2.43 vs 9.62±1.70,P<0.05),B组平均获卵数要高于A组但两组无统计学意义(10.43±7.94 vs 9.00±6.98,P>0.05),两组之间的HCG注射日血清E2与LH水平及内膜厚度、Gn用量、受精数、受精率、卵裂数、卵裂率、妊娠率均无显著性差异(P>0.05)。结论:GnRH-A是卵巢功能低下患者的促排卵治疗可行方案。

The GnRH-antagonist in control ovarian stimulation of female with ovarian function decreased

Objective:To compare the difference between GnRH-antagonist(GnRH-A) and GnRH-agonist(GnRH-a) in controlled stimulation of female with ovarian function decreased.Methods:Group A: A total of fifty three controlled ovarian stimulation cycles received GnRH-a(GnRH-a and HMG/rFSH) protocol;Group B: A total of thirty controlled ovarian stimulation cycles received GnRH-A(GnRH-A and HMG/rFSH) protocol.Serum LH and E2,endometrial thickness on day of HCG administration,total gonadotrophin consumption,total stimulation duration,number of oocytes per retrieval,fertilization rate,cleavage rate,implantation rate and pregnancy rate were compared.Results:A significant difference was found in the total stimulation duration between Group B and Group A(10.87±2.43 vs 9.62±1.70,P<0.05).The number of oocytes per retrieval in group B was higher than that in Group A(10.43±7.94 vs 9.00±6.98),but there was no significant difference between the two groups(P>0.05).There was no significant difference(P>0.05) between the two groups in serum LH and E2,endometrial thickness on day of HCG administration,total gonadotrophin consumption,fertilization rate,cleavage rate,implantation rate and pregnancy rate.Conclusion:GnRH-A protocol is an alternative method which will be used in controlled stimulation of female with ovarian function decreased.
